  • Client: Name/ID • DOB
  • Date: YYYY‑MM‑DD • Start–End Time
  • Location: Clinic / Home / School / Telehealth
  • RBT: Your Name • Supervisor/Caregiver Present

2. Targeted Goals or Programs

List your objectives clearly. For example:

  • “DTT manding: 2-word phrases”
  • “Chaining handwashing (4 steps)”
  • “Reinforcement via token board”

3. ABC Data Summary

Capture key behavior incidents with a concise table:

4. Intervention Details

  • Prompt level: Full physical → partial

  • Graph updated? Yes / No

6. Client Engagement & Progress

Keep it factual:

  • Skill acquisition: “Increased independent mands from 40% to 60%”
  • Behavior change: “Tantrums reduced from 5 to 2 instances”
  • Generalization: “Used skill with parent at session end”
  • Engagement: “Attended 90% of session, remained seated”

7. Adjustments & Challenges

Record any session deviations:

  • Issues: “Required extra prompts during step 2 chaining”
  • Adaptations: “Switched to high-preference edible immediately.”
  • BCBA feedback: “Supervisor recommended modeling firs.t”

8. Caregiver Input (if present)

Briefly note their observations or concerns:

9. Plan for Next Session

Be proactive and structured:
